UCP Skilled Nurses
Skilled Nurses (or SN) are licensed nurses that check temperature, pulse rate, respiratory rate, blood pressure, blood sugar, assess pain and management, check capillary blood sugar level, perform general body assessment as well as to observe instability of the client’s medical status. They perform certain medical procedures within their scope of nursing practice. They also offer health education specific to the client’s condition.
UCP Physical Therapists
Physical Therapists (or PT) provide rehabilitation treatment to restore maximum level of physical capabilities such as to strengthen muscles to assist and educate clients to walk better, prevent falls, use Durable Medical Equipment and assistive devices properly, and use treatments to manage pain. They also provide home safety and caregiver education.
UCP Speech Therapists
Speech Therapists (or ST) provide treatments for the management of communication disorders and education for the patient and/or family concerning methods of speech language improvement.
UCP Occupational Therapists
Occupational Therapists (or OT) provide activities designed to improve physical and psychological functioning and teach the patient and his/her family in the use of adaptive equipment.
UCP Home Health Aide
Certified Home Health Aides (or CHHA) offer assistance in bathing, personal care, and light housekeeping.
UCP Medical Social Worker
Medical Social Workers evaluate the client’s emotional and mental state and makes suggestions, assists in the processing of forms through Medicare or other insurances, assists in securing Meals-On-Wheels or transportation, assess the caregiver’s continued ability to provide care in finding help in the home (i.e., instituting eligibility for in home support, Medi-Cal and SSI, Senior Citizen Tax Benefit, etc.).
UCP Durable Medical Equipment
Our staff is able to assess and make arrangement for obtaining medically necessary equipment as prescribed by physician. Durable Medical Equipment (DME) such as hospital beds wheelchair, commodes, shower chairs, crutches, canes, tube feeding equipment, oxygen, suction machines, pulmoaide, etc., that are available under the patient’s insurance plan and may arrange for the delivery of such items to the patient’s house.
UCP Pharmacy
Our staff is able to coordinate with the pharmaceutical needs of the patient such as I.V. Therapy, inject able medications, diabetic supplies, wound supplies, etc.
UCP Laboratory Work
Our staff is able to coordinate laboratory work ordered by the patient’s physician.
UCP Other
We are able provide Interpreter Service for current patient with impairments in communication, e.g., deaf, hearing or speech impaired, or Limited English Proficient (LEP) persons through the Language Line Services.
