How do I ask about home health care?
You can call us toll-free at (800) 797-2015 or at (949) 788-0907. Our team can discuss your questions and explain what information is needed to explore services. You can also ask your primary care provider (PCP) whether you are eligible for home health care services through a home health agency (HHA).
Can I refer a patient or family member?
Yes. Call our office for referral instructions. Please contact us before sending patient records so we can explain how to share them appropriately.
Will my insurance cover services?
Coverage depends on your plan, the services needed, and eligibility requirements. Contact our office to discuss coverage verification before care begins.
Where is your office?
Our office is at 1 Peters Canyon Road, Suite 160, Irvine, CA 92606. Published office hours are Monday through Friday, 9:00 a.m.–5:30 p.m.
