POSITION: ADMINISTRATIVE SUPERVISOR
POSITION DUTIES: Assist management team for scheduling of patient appointments for home health care, supervision and appointment of staffs, managing insurance claims, keeping patients’ records and updating clinic’s database, and management of clinic’s financial aspects
QUALIFICATIONS: Any Associate degree
RATE OF PAY: $65,458 per year
HOURS: 40 hours per week (Full-time & Permanent Position)

UCP Skilled Nurses
Skilled Nurses (or SN) are licensed nurses that check temperature, pulse rate, respiratory rate, blood pressure, blood sugar, assess pain and management, check capillary blood sugar level, perform general body assessment as well as to observe instability of the client’s medical status. They perform certain medical procedures within their scope of nursing practice. They also offer health education specific to the client’s condition.
UCP Physical Therapists
Physical Therapists (or PT) provide rehabilitation treatment to restore maximum level of physical capabilities such as to strengthen muscles to assist and educate clients to walk better, prevent falls, use Durable Medical Equipment and assistive devices properly, and use treatments to manage pain. They also provide home safety and caregiver education.
UCP Speech Therapists
Speech Therapists (or ST) provide treatments for the management of communication disorders and education for the patient and/or family concerning methods of speech language improvement.
UCP Occupational Therapists
Occupational Therapists (or OT) provide activities designed to improve physical and psychological functioning and teach the patient and his/her family in the use of adaptive equipment.
UCP Home Health Aide
Certified Home Health Aides (or CHHA) offer assistance in bathing, personal care, and light housekeeping.
UCP Medical Social Worker
Medical Social Workers evaluate the client’s emotional and mental state and makes suggestions, assists in the processing of forms through Medicare or other insurances, assists in securing Meals-On-Wheels or transportation, assess the caregiver’s continued ability to provide care in finding help in the home (i.e., instituting eligibility for in home support, Medi-Cal and SSI, Senior Citizen Tax Benefit, etc.).
UCP Durable Medical Equipment
Our staff is able to assess and make arrangement for obtaining medically necessary equipment as prescribed by physician. Durable Medical Equipment (DME) such as hospital beds wheelchair, commodes, shower chairs, crutches, canes, tube feeding equipment, oxygen, suction machines, pulmoaide, etc., that are available under the patient’s insurance plan and may arrange for the delivery of such items to the patient’s house.
UCP Pharmacy
Our staff is able to coordinate with the pharmaceutical needs of the patient such as I.V. Therapy, inject able medications, diabetic supplies, wound supplies, etc.
UCP Laboratory Work
Our staff is able to coordinate laboratory work ordered by the patient’s physician.
UCP Other
We are able provide Interpreter Service for current patient with impairments in communication, e.g., deaf, hearing or speech impaired, or Limited English Proficient (LEP) persons through the Language Line Services.

My dad has been in and out of the hospitals for the past 8 months and after his initial discharge, we were assigned to a care provider based in L.A. by the care facility. They were always either late or miss the appointments and my Dad situation got worse under their care to a point that had to go back to the hospital. By chance and through a 3rd party, I was referred to this company. After switching to Unified Care, I could not believe the difference between the service they offered compared to the previous one. From day one, one of their Nurses, (Mrs. Manijeh) rightfully diagnosed (later verified by the doctor) that a back wound he had been treated for, was the wrong diagnosis for that wound. It took one visit from her versus 4 months under care of the other company's nurse. After that, the other nurses, Mrs. Mehry and specially Mrs. Mahin who has been attending to his post-surgery wound vacuum and is getting better every day. She even came on the Memorial day holiday, because she did not want to miss his routine change of his wound. This company and their staff are like angels That I needed to help me with my Dad's situation. I should also mention the administrator person, Mrs. Shohreh; who is always great and pleasant and arranges all the appointments without any hassle. Something I never experienced with the other company. If you are looking for a professional and reliable Home health services, don't waste your time by going anywhere else. I absolutely love her as a person and as my mom’s nurse. Her patients are important to her and her level of professionalism & compassion is rare among healthcare providers. Nurse Mahin saved my aunt’s life at least two times with her incredible knowledge where she acted very quickly and vigilantly. She had a very complex medical history and Nurse Mahin was the perfect nurse to care for her before she passed away few months ago.
